Switch Care Plans

If your account has multiple care plans, you will need to switch between each health record to view the exercises.

1. Once logged in with your existing account, press the ≣ icon.

2. Select Settings from the list.

3. Select My Health Records.

4. The current care plan is designated by a checkmark. You can use the date of birth or clinic name to identify the care plan you want to open.
